How to decide on your dream trip?

I have always dreamed of going on a big trip, but I can’t bring myself to take this step. Something always stops me: work, financial issues, and just fear of the unknown. Have any of you tried to realize your dream and go on a trip? What were your first steps and how did you deal with doubts?
